Sometimes patients have an unfortunate outcome through no fault of the surgeon. However, occasionally a preventable surgical error occurs, resulting in life-changing consequences to the patient. A nerve is cut. The bowel is perforated. A suture is misplaced. A surgical sponge or an instrument is left in the patient. The list goes on. One preventable surgical mishap can result in catastrophic loss.
